If you no longer have the porting form, our customer service (0800 66 88 66) will be happy to send you a new one.
Regarding the costs that have already been charged, I have just credited your account with the costs for May (67.63 CHF) and June (59.90). The credit will be visible in MyUPC in about 48 hours. After your number has been ported successfully, we will credit the remaining time until the definitive connection.
