what does this mean?

On startup I get the following messages:

 

"Switching to amplifier."

 

"Failed! TV speakers active"

 

Is my box defective ?

That sounds like an HDMI-CEC error.  The message says that your AVR is turning on and wants to use its speakers for the sound, but either isn't turning on quickly enough or isn't registering its features properly, and hence the sound is going via passthrough to your TV and using the TV speakers. 

 

Do you have your UPC box connected to an AVR and then the AVR connected to the TV?

 

If you don't have an AVR then you can ignore the message as it's doing no harm, but do the below anyway.

 

Power everything off completely, wait around 30 seconds, then power everything back on.  My "power off/on" I mean at the socket, not standby.

 

That should reset CEC.
